If you have a fever of 100.4 or above, are coughing, or have other flu-like symptoms, please call your medical office location listed below before you come in.

If you have traveled outside the country in the last 14 days, please call before you come in.

If you or someone in your household has been diagnosed with COVID-19, or have had close contact with anyone diagnosed with COVID-19, OR IF YOU HAVE BEEN IN CONTACT WITH SOMEONE WHO HAS HAD TESTING FOR COVID-19 AND IS WAITING FOR RESULTS, please call before you come in.

No children age 14 or less is allowed in the clinic.


If you need assistance, there can be no more than 1 caregiver or family member in the clinic with you.
Family, friends, and escorts must NOT remain in our waiting room, during your treatment.
Please be dropped off and picked up from your appointments whenever possible. You are here for an appointment which is a medical necessity. Please help us to keep you and our staff safe by minimizing social contact.
