BECOMING A PATIENT
Los Angeles Cancer Network has been delivering excellence in cancer care for more than 30 years. Becoming our patient is easy, simply call (323) 760-9757 or Request an Appointment
Once in contact with our New Patient Administrator, they will assist with the following:
- Insurance verification and pre-authorization
- Schedule the initial appointment
- Provide all registration forms
- Answering any questions about your initial visit
Preparation and documentation:
- Please arrive at least 30 minutes before your appointment time to allow for completion of paperwork and verification of coverage.
- Plan to spend 1 to 2 hours at your first visit.
- Bring the following to your appointment:
- Picture ID
- Insurance Card
- Medication List
- Pre-authorization (if needed)
- New Patient Forms
Note: We accept most major insurance plans. Pre-authorization may be required for many of the treatments administered by Los Angeles Cancer Network. If you are unsure about your coverage, you do not have health insurance, or any of the treatments we recommend are not covered by your insurance, please call our office at 323-254-0046 and we will be happy to assist you. We may be able to help you with costs or make special financial arrangements.
